For the processing of personal and health data
I hereby give my unconditional consent, for an indefinite period, to the following,
- namely that MicroBiome Bank Kft. (H-1118 Budapest, Ménesi út 104. II. ép al, tax number: 26167969-2-43, represented by: Dr. Belatiny-Kenéz Attila), a healthcare provider – hereinafter the Data Controller – may, for the purposes of performing its tasks in connection with the services it provides, record, register and process in its database, to the extent necessary, my voluntarily provided personal Data (my identification and contact data: name, telephone number, e-mail address and other identifiers) and the health data revealed in relation to me, and may transfer these to the EESZT system on the basis of statutory provisions.
- I further give my consent to, and authorise, the Data Controller to have full access to, become acquainted with and process the health data, information and documents relating to me that have been uploaded to and are available in the EESZT system, during the period of my participation in the donor programme and for a further two months following the donation of the last donation. In order to enable such full access, I shall without delay ensure that, on the "Self-determination" (Önrendelkezés) interface available on the public portal at https://www.eeszt.gov.hu, in the menu "Status regulated by a composite disposition" (Összetett rendelkezéssel szabályozott állapot), I grant the duly authorised member of the Data Controller access to documents "originating from anyone" as regards the source of the document and "of any type and category" as regards the content of the document.
- I consent to the Data Controller using, in an anonymous manner and for scientific and research purposes, the experience derived from the above data and documents and from the use of the donation.
- I declare that the personal data provided by me are complete, and I undertake to inform the Data Controller without delay of any changes occurring during the term of the cooperation.
- I acknowledge that the data processing carried out on the basis of the consent contained in this statement is governed by the Data Controller's Policy and by the data protection legislation in force from time to time, in particular EU Regulation 2016/679, the General Data Protection Regulation (GDPR), Act CXII of 2011 on the Right of Informational Self-Determination and on Freedom of Information (the Information Act), Act CLIV of 1997 on Healthcare, and the provisions of Act XLVII of 1997 on the Processing and Protection of Health Data and Related Personal Data.
- I have the right to request information from the Data Controller at any time in connection with my personal data, or the rectification thereof. I am entitled to withdraw my consent at any time, with immediate effect and without giving reasons, by means of a written statement sent to the Data Controller, which may be done by post (1118 Budapest, Ménesi út 104. II. ép al) or in the form of an electronic message (info@MicroBiomeBank.org).
- I acknowledge that the withdrawal of consent does not affect the lawfulness of the data processing based on the consent and carried out prior to the withdrawal, nor the Data Controller's data retention obligation arising from its statutory obligations.
- I further acknowledge that the restriction of the processing of my own personal data or of the health data revealed in the course of the examinations, or the refusal to provide or the withdrawal of such data, may prevent cooperation with the Data Controller for the performance of healthcare tasks.
- In the event of a suspected breach of the data processing rules, I am entitled to lodge a complaint with the Hungarian National Authority for Data Protection and Freedom of Information (NAIH) (1125 Budapest, Szilágyi Erzsébet fasor 22/c; telephone: +36 1 391 1400, e-mail: ugyfelszolgalat@naih.hu, web: www.naih.hu), and furthermore to bring the matter before a court pursuant to Section 22 of the Information Act.
